Please click on the links below to download the appropriate (Sponsor or Player) registration form.  The registration forms are available in both a fillable doc to download to your computer or a pdf for you to print and complete.  When your registration form is completed, please email to sossenrichmentfundsociety@gmail.com. and etransfer your payment to this same email address.

You can also send the completed registration form via Canada Post, along with your payment, to the address on the registration form.   In the event of a postal disruption, please email us to arrange for  delivery or pick up of payment if required.
